The tools listed below are easy to use. Just check out the examples (where available), and take a 'test drive' to get the feel of it.
Then all you have to do is plug in your information and click a button to make the code. Placing it on your website is as easy as copy/paste.
Everything here is free to use, all I ask is for a small link from your site to mine.

AnimationOnline.com - Not as many designs to choose from, (yet) but its quick and easy to use. You can do the full-size banners, buttons or box displays - and they don't add any self-promotion to the finished product.
Logo Generator - A fairly basic 3-D, textured-text logo generator... plus a half-dozen or so editable buttons. However, the final product looks ok and is available in different formats.
